Hodos Health is a health-tech application company focused on improving the journeys of patients with chronic conditions and the performance of the clinics that care for them. Our inspiration comes from our co-founder who has multiple chronic conditions, her care team, as well as the patients and clinicians we’ve seen challenged by this growing area of healthcare. Our solution is built on proven performance tools used in industry, which we intelligently automate to eliminate labor intensive tasks in care coordination and staff scheduling. And importantly, we do this behind the scenes — reducing staff interactions with EHR, Practice Management, and calendaring systems. Our outcomes support the Quadruple Aim of Healthcare: reducing costs; improving population health, patient experience, and care team well-being and productivity.
