Spiras Health delivers high-touch, specialized care management to patients who are at an elevated risk of an emergency event or hospitalization due to complex, chronic illnesses, complicated by social determinants of health and other barriers to care. We make care accessible through a multi-modal approach, providing services at home, where patients most want to be, and augmenting those services with telehealth, digital communications, and remote patient monitoring. This approach accommodates the potential uncertainty of events such as a pandemic or a natural disaster, minimizing the potential of missing much needed care. Health plans provide Spiras Health to qualifying patients as a benefit and at no additional cost to the patient.
